Thin endometrium and low ovarian reserve

Two different problems share this page because both are common questions in fertility care. Low ovarian reserve means that the number of eggs left in the ovaries is lower than expected for age. A thin endometrium means the uterine lining is thinner than needed for an embryo to implant. At EVE Women and Fertility Clinic in Gunjur, Bangalore, Dr. Swati Shree explains both honestly: what the numbers mean, what they do not mean, and what can be done.

At a glance

  • Low ovarian reserve

a low AMH or antral follicle count for your age. It affects how many eggs can be collected, and how much time you have.

  • AMH

a blood test, valid at any day of the cycle, that reflects the number of small follicles

  • Antral follicle count (AFC)

follicles counted on a scan early in the cycle

  • Thin endometrium

often a lining under about 7 mm around ovulation or before transfer, though the cut-off varies

  • Causes of a thin lining

low estrogen, scarring (Asherman syndrome), chronic endometritis, past surgery or infection, and genital tuberculosis

  • Treatment

    matched to the cause

Low ovarian reserve

Understanding the tests. AMH and AFC estimate how many eggs remain. They tell you about number, not quality. A low value suggests that fewer eggs may be collected in IVF and that time may be shorter, but it does not by itself predict that a pregnancy is impossible. A high value, such as in PCOS, is not a measure of better fertility either.

Causes: age, genetic factors, surgery on the ovaries, endometriosis, chemotherapy or radiation, some autoimmune conditions, smoking, and sometimes no known cause.

What helps, honestly.

  • Acting early. With low reserve, time matters more than in most situations.
  • Considering IUI or IVF sooner if you are over 35, or if reserve is very low.
  • Discussing egg freezing, if you do not plan pregnancy soon. With low reserve, fewer eggs are collected per cycle, so expectations must be realistic.
  • Avoiding unproven supplements and add-ons, which have little or no evidence. Dr. Swati explains the evidence for any option you ask about.
  • Stopping smoking, and keeping medical conditions under control.

Premature ovarian insufficiency

When periods stop before 40, with high FSH on tests, it is called premature ovarian insufficiency. It needs a medical evaluation, hormone therapy for health, and a discussion of fertility options. See Menopause and perimenopause.

Thin endometrium

The endometrium needs to be thick enough, and well-formed, for an embryo to implant. A thin lining can be seen in women with and without fertility problems. A lining below about 7 mm at the time of ovulation or embryo transfer is commonly considered thin, but there is no single threshold.

Causes

  • Low estrogen
  • Scarring from infection or procedures (Asherman syndrome)
  • Chronic endometritis, an inflammation of the lining
  • Genital tuberculosis
  • Poor blood flow
  • Medicines used in some treatment cycles

Evaluation. A transvaginal scan across the cycle, and, when suspected, hysteroscopy, a tissue biopsy, or tests for infection and tuberculosis.

Treatment

  • Treat the cause: antibiotics for chronic endometritis, hysteroscopic division of adhesions, treatment of tuberculosis if found
  • Estrogen priming in a prepared embryo transfer cycle
  • Freezing embryos and transferring in a later, better prepared cycle
  • Other measures, such as platelet-rich plasma, growth factors, or low-dose aspirin, have limited evidence, and are discussed only with clear explanations of what is known

When to see a doctor

  • AMH reported low, or antral follicle count low on a scan
  • Periods stopping, becoming very light, or irregular, especially before 40
  • A thin lining reported on scan, or failed embryo transfers
  • Trying to conceive over 35, with limited time

FAQs

What is a normal AMH level?

There is no single normal level. AMH falls with age, and values are compared with the typical range for your age and the laboratory. Very low values suggest reduced reserve, and very high values are seen in PCOS. Dr. Swati reads your AMH with your age, scan and history.

Does low AMH mean I cannot get pregnant?

No. AMH reflects egg number, not quality, and women with low AMH can conceive naturally. It may mean that fewer eggs respond to stimulation, and that time is shorter. Acting earlier matters. Dr. Swati advises on options such as timed attempts, IUI or IVF.

Can I increase my AMH?

AMH cannot be increased in any proven way, since it reflects eggs already present. Supplements and treatments sold for this purpose have little evidence. Healthy habits, such as stopping smoking, support general health. If you plan pregnancy, an early evaluation helps more.

What is a thin endometrium, and can it be treated?

A thin endometrium is a lining that is thinner than needed for implantation, often under about 7 mm. Causes include low estrogen, scarring and infection. Treating the cause, estrogen support, and sometimes freezing embryos for a better prepared transfer can help.

Can I freeze eggs if my reserve is low?

Sometimes. With low reserve, fewer eggs are collected in each cycle, and more than one cycle may be needed. Egg freezing may still be worth discussing, especially if you are young, but the benefit is less certain. Dr. Swati gives a realistic view.

Does a thin lining always cause implantation failure?

No. Pregnancies can occur with a thin lining, though the chance is lower. A thin lining is one factor among several. Dr. Swati checks for treatable causes, such as adhesions or infection, before advising a plan.
